Company Cited for Multiple Violations Following a Worker Fatality

OSHA opened an investigation after a six-ton piece of stainless steel became dislodged from a crane and struck an employee at a Pennsylvania manufacturing facility. As a result, 42 serious and four repeat violations were cited. Serious violations include alleged failure to ensure that a swivel hook on an overhead crane was equipped with a safety latch and alleged operation of an overhead crane by workers who were insufficiently trained. Other alleged violations include operation of a crane beyond its rated load capacity and failure to conduct a personal protective equipment (PPE) workplace assessment. Alleged repeat violations include failure to provide machine guarding and required training in hazardous energy sources. Proposed penalties in this case total $119,400.
