Load Securement – Securing Pipes and Barrels Fatality File

The incident involved a worker, who was working with pipes containing chemicals. The worker was found to be intoxicated and collapsed on the work site. Despite attempts to revive him, he succumbed to his injuries and was pronounced dead at the scene.

Investigation Findings: Our investigation revealed that the cause of the worker’s intoxication was due to improper securing of the pipes containing chemicals. The pipes were not properly labeled or secured, which led to the worker unknowingly coming into contact with the chemicals. The worker was not wearing proper personal protective equipment at the time of the incident, which further increased his exposure to the chemicals.

Contributing Factors: The following factors contributed to the incident:

  1. Lack of proper labeling and securing of the pipes containing chemicals.
  2. Failure to ensure that workers were trained on the proper handling and use of the chemicals.
  3. Failure to ensure that workers were wearing proper personal protective equipment while working with the chemicals.

Recommendations: To prevent future incidents, the following recommendations are made:

  1. Ensure that all pipes containing chemicals are properly labeled and secured.
  2. Provide proper training to all workers on the handling and use of chemicals.
  3. Ensure that all workers are wearing proper personal protective equipment while working with chemicals.
  4. Conduct regular safety audits to identify and address potential hazards.
