Worker Struck by Equipment After Failing to Hear Warning Signal

A worker was operating in a high-noise environment where heavy equipment was moving nearby. Due to elevated noise levels, the worker did not hear warning signals, alarms, or verbal communication from coworkers.

While focused on the task, the worker stepped into the path of moving equipment and was struck and fatally injured. Investigations identified limited situational awareness caused by noise exposure as a contributing factor.
