Steel Load Falls Off Crane

An overhead crane operator died when a bale of steel slipped off the C-hook and hit him.

He had positioned the bale incorrectly on the C-hook, so it was not balanced and centered. Ordinarily he was skilled at his work, but investigation into this fatality indicated he had used poor judgment because he was impaired by alcohol.

Fatalities such as this are one reason use of alcohol is prohibited in the workplace. No matter now talented and well trained the worker, slowed reflexes and impaired judgment can cause fatalities.

Everyone knows how dangerous it is to drink and drive. It is just as dangerous to operate industrial machinery after drinking alcohol.
