Texas Refinery Cited for Alleged Serious, Repeat Violations Issued $237,500 in Proposed Fines

An inspection of a crude oil refinery in Big Spring, TX has resulted in OSHA citations for three alleged repeat violations associated with process safety management. Twenty-seven serious violations were also issued for alleged hazards associated with process safety management, confined spaces, control of hazardous energy, portable fire extinguishers, emergency response and evacuation, electrical deficiencies, chemical storage and walking/working surfaces.
