Labelling Chemical Containers Fatality File

Worker Dies After Ingesting Chemical from Unlabeled Bottle

In September 2020, a 59-year-old employee at CPAC Contracting Services, Inc. in New York tragically lost his life due to chemical poisoning. While using the restroom, the worker drank from an unmarked bottle, believing it contained water. Unbeknownst to him, the bottle held hydrofluoric acid—a highly toxic and corrosive substance. The ingestion led to fatal internal injuries, resulting in his death.

This incident underscores the critical importance of proper chemical labeling in the workplace. The absence of clear labeling on the container directly contributed to the fatal mistake. Employers are mandated to ensure that all hazardous chemicals are appropriately labeled, and employees must be trained to recognize and understand these labels. Such measures are vital to prevent similar tragedies and to maintain a safe working environment.
