Fall from Rain-Slick Roof

A 45-year-old roofer died after falling 30 feet from a rain-slicked, 5/12 pitch plywood roof deck. The incident occurred while a crew of six was installing roofing material in light rain. When the victim realized there was no anchor point available for him to connect his fall protection harness, he descended the ladder, retrieved an anchor, and returned to the roof to install it on the ridge. While preparing to install the anchor, he lost his footing and slid about 12 feet down the wet plywood deck. He attempted to grab the roofing underlayment to stop his descent but failed, falling off the edge, striking a steel storage container, and landing on the ground.
